I authorize Otter Lake Animal Care Center to treat such illness in a reasonable manner, using only necessary examinations, tests, and treatments to remedy the problem. I also agree to pay the necessary fees for these services.2. I require notification prior to any treatment being initiated and agree to pay the charges to effect such communication. If I cannot be reached, I authorize reasonable treatment.OWNER RELEASE Otter Lake Animal Care Center (OLACC) is to use all reasonable precautions against sickness, injury, or escape of my pet(s). The clinic and staff will NOT be held liable for any problems that develop provided that reasonable care and precautions are followed. I realize that I am responsible for all agreed upon charges and that payment in full is due at the time of discharge. If I neglect to pick-up my pet(s) within five (5) days of the date above and do not notify OLACC within that period of time, OLACC may assume that the pet(s) are abandoned. OLACC is then authorized to dispose of the pet(s) in whatever manner they deem best or necessary. Abandonment does NOT release me of my obligation for all charges incurred. OLACC and its staff will NOT be held liable for any problems resulting from multiple pets being boarded together at the owner’s request. OLACC does NOT recommend that multiple pets be boarded together in the same cage or run.
